تزداد احتمالات معاناة المرضى المُصابين بداء السُّكَّرِي
من مضاعفاتٍ خطيرة جرَّاء الإصابة بمرض «كوفيد-19»، ويحاول العلماء فهم الأسباب
التي تجعل فيروس كورونا المُستَجَد أشدَّ وطأة لديهم، كما يبحثون كيفية الحد من خطورته.
خلصت إحدى الدراسات التي نُشِرَت في دورية «ديابيتس كير»
Diabetes Care، للباحث جاستن جريجوري من قسم إيان إم. بور لطب الغدد الصماء والسُّكَّرِي عند الأطفال
في كلية الطب بجامعة فاندربيلت في مدينة ناشفيل، بالاشتراك مع زملاء آخرين، إلى أن شدة
الإصابة بمرض «كوفيد-19» تتضاعف ثلاث مرات بين مرضى السُّكَّرِي1.
فارتفاع نسب الجلوكوز في الدَّم يتيح لفيروس كورونا المُستَجَد التغلغل في الخلايا

وصولاً إلى الأوعية الكبيرة المؤدية إلى الشريان التاجي. ويُضيف قائلًا: "إذا كان المرء لديه بالفعل استعدادٌ للإصابة
بأمراض الأوعية الدَّموية، ثم أُصيب بمرض «كوفيد-19»، فلن يستغرق الأمر كثيرًا من الوقت قبل إصابته بالجلطات
التي تمثِّل إشكالية كبيرة أو حتى تصبح تلك الشُّعَيرات الدَّموية مُرشِّحة مما يسمح للسوائل بالدخول إلى الرئتين".
وفقًا لجريجوري، ثمة أدلةٌ كافية تُشير إلى أن مرض «كوفيد-19»

التَّقييم، أخذوا بعين الاعتبار أيضًا عوامل الخطورة الأخرى. يقول جريجوري: "إذا دخلت وحدة الرعاية المركزة في
مدينةٍ بها كثير من حالات الإصابة بمرض «كوفيد-19»، فقد تظن أن أغلب المرضى الذين يعانون بشدّة جرّاء الإصابة
بمرض «كوفيد-19» مُصابون بداء السُّكَّرِي من النوع الثاني. ليس هذا بسبب أن الأشخاص المصابين بالنوع الأول
أقل عرضة للخطر، وإنما يرجع إلى قلة عددهم نسبيًا، ولأنهم في أغلب الأحيان ينتمون إلى شريحةٍ عُمْرية أصغر سنًا".
ميتفورمين: عقار قديم.. رؤى جديدة
تكشف دراسات جديدة أن مرضى السُّكَّرِي الذين يتناولون عقار «ميتفورمين» metformin
-وهو خط العلاج الدَّوائي الأوَّل
لعلاج داء السُّكَّرِي من النوع الثاني- يكون تطوُّر المرض لديهم أخف وطأةً عند إصابتهم بمرض «كوفيد-19».
يقول عبد الله السلامة، من قسم طب الغدد الصماء والسُّكَّرِي والتَّغذية في المستشفى
الجامعي بأميان في فرنسا، والذي أشرف على دراسة بخصوص عقار ميتفورمين نُشِرَت في دورية «ديابيتس ميتاب»
Diabetes Metab: "استُخدِم عقار ميتفورمين في أوائل القرن العشرين لعلاج الإنفلونزا". ويوضِّح السلامة أن عقار «ميتفورمين» له بعض الخصائص المضادة للميكروبات، والمضادة للالتهابات، والمُثبِّطة للمناعة، والمضادة للتليُّف2. ويضيف: "كل هذه الأمور مُهِمة عند التعامل
مع مرض «كوفيد-19» لأنه مرض ميكروبي تصاحبه عاصفة السيتوكين".
ورغم أن مستخدمي عقار «ميتفورمين» يكونون أفضل حالًا من غيرهم
بعد الإصابة بالعدوى، لا يزال من السابق لأوانه استقراء ما هو أبعد من وجود ارتباطٍ بين استخدام عقار «ميتفورمين»
والتخفيف من بعض أعراض مرض «كوفيد-19» الشديدة. ومن المستبعَد كذلك أن يكون لإعطاء عقار
«ميتفورمين» للمرضى الذين لم يتناولوا العقار قبل دخول المستشفى أي فوائد، أو أن يؤدي إلى تحسُّنٍ ملحوظ.
يقول السلامة: "ما نعرفه أن عقار «ميتفورمين» مفيدٌ للمصابين بداء السُّكَّرِي في تخفيف معاناتهم الشديدة من مرض «كوفيد-19»،
ولكن إذا أعطينا المريض عقار «ميتفورمين» في المرحلة الحادة من المرض، فهل سيكون مفيدًا؟ نحن بحاجة
إلى إجراء دراسة منضبطة مُعشَّاة للإجابة عن هذا السؤال. فكل ما لدينا حتى الآن دراسات رصدية".
ويضيف السلامة أنه لا توجد أدلةٌ تُشير إلى أن عقار «ميتفورمين» يُمكِن أن يقي من العدوى.
يحاول العلماء أيضًا تحديد ما إذا كانت ثمة صلة بين مرض «كوفيد-19»
واضطرابات الجلوكوز الأيضية أم لا. فقد أنشأت مجموعة دولية من الباحثين في مرض السُّكَّرِي سجل «كوفيدياب»
