Munir's disease and its unknown causes.
Before entering diseases, let us look at the anatomy of the human ear, which consists of three sections: the outer ear (the pinna and the external auditory canal), the middle ear, the eardrum and the auditory ossicles, the eustachian tube and the mastoid bone) the inner ear: it consists of the labyrinth, which is filled with a fluid called peripheral lymph. The bony labyrinth consists of semi-automatic ducts, vestibule, and cochlea, the majority of which is connected to the brain by a channel called the cochlear aqueduct. The majority consists of the semicircular ducts, the vestibule, and the cochlea. The greater proportion is connected to the brain by a channel called the cochlear aqueous canal through which the peripheral lymph connects with the cerebrospinal fluid.
Therefore, the structure of peripheral lymph is similar to that of cerebrospinal fluid and extracellular fluid, as the main ion in it is sodium. As for the membranous labyrinth, it is located within the bony labyrinth. The membranous labyrinth is filled with the inner coil and resembles the composition of the fluid inside the main stray cells, which is potassium. As for the three semicircular canals, they are placed perpendicular to each other. These channels end with balance-sensitive cells that connect with the vestibular nerve, which runs with the spiral nerve in the internal auditory canal to enter the brain. As for the function of the vestibule, it is to help maintain balance through reflexes that put the body in a position that protects it from falling. If the head is tilted to one side, the limbs and trunk take an appropriate position so that the person does not fall, and information about the position of the body parts is transmitted to the brain through three ways: the eye, the ear, and the proprioception. . The eye gives information by direct vision, and the deep sense gives information about the tension that occurs to the tendons, joints, and the ear, and it gives information about the position of the head in movement and stillness. We will move on to the definition of vertigo, which is a feeling of the rotation of the body or the rotation of things around the body, i.e. a false feeling of the presence of a movement of milk between the person and the environment, and it is the result of a defect in the system that maintains the balance of the body, and it is accompanied by a feeling of nausea, sweating, and heart palpitations. One of the most important diseases that lead to dizziness is Meniere's disease, which is hypertension in the internal coil that includes the membranous labyrinth, which leads to the membranous labyrinth, which leads to the expansion and swelling of the cochlea and its protrusion within the vestibule. The real causes of this disease are still unknown, but nevertheless, it has been found that disturbance of water metabolism, retention of sodium ion in the body, sensitivity to histamine, vascular spasms and hormonal disturbances all lead to a change in the biochemical composition of the endolymph, leading to the peripheral lymph to turn it into the endoskeleton, leading to an increase in pressure in it with a change in its nature. chardia. It is mostly a unilateral disease, i.e. it usually affects one ears, but there are cases that affect both ears and it affects both sexes with little satisfaction for males as one of its most important symptoms. Vertigo, which is the main symptom of this disease and comes in the form of episodes of sudden severe vertigo, which lasts from minutes to hours. Nausea and vomiting accompanies and then ends, and it varies from one episode to another, as it comes in groups, and the patient suffers from several close episodes and then leaves him until the patient thinks that he is completely cured, then the seizures return to him again and nystagmus (rapid eye movement) may appear and be in the opposite direction to the affected ear. Hearing loss: It is of the sensorineural type and it is gradual with the recurrence of the seizure and it becomes permanent with the presence of the phenomenon of alertness, which is the patient’s discomfort from loud sounds. Tinnitus: It is low in frequency at first and then becomes continuous. The appearance of tinnitus and hearing loss may follow the appearance of vertigo for months or even years. Sense of heaviness in the ear: The patient feels a sense of heaviness and pressure in the affected ear. Mounir's disease is accompanied by additional symptoms, which are nausea, vomiting, and even diarrhea, as well as pallor, cold sweats and headaches. As migraine is associated with this disease in 20% of cases, the patient complains of a serious brain disease, and the diagnosis depends on the correct interrogation, the clinical history, and the appearance of the four symptoms combined during the seizure. The audiological tests depend on the audiogram, where it shows a lack of hearing due to an injury to the inner ear, or the real causes of this disease are still unknown, but in some cases, several reasons can be revealed, including: Chronic middle ear with infections of the membrane - immune disorders The definition is similar to infections of the vestibule and the vestibular nerve, proper postural vertigo, and Lytic sclerosis. Or treatment is pharmacological and surgical. Pharmacological treatment: It is by reassuring the patient and explaining the problematic nature of the disease with giving him sedatives with anti-vertigo and vomiting, as well as giving him vasodilators with advice to the patient to stay away from eating salt and it is advised to stay away from smoking. Surgical treatment: it is resorted to in a few cases and in the event of failure of drug treatment, including decompression of the endolymph, rupture of the vestibule, cutting the vestibular nerve, or giving some drugs that inhibit the vestibule such as methammycin. But most surgical treatment results are not encouraging. In sum, Munir’s disease is not a malignant disease, but it is very annoying to the patient, as it prevents him from working and leads him to severe fear. Therefore, the doctor must be understanding of the situation and reassure the patient to reach the maximum degree of treatment.

قبل الدخول بالأمراض نلقي نظرة على تشريح الأذن البشرية التي تتألف من ثلاث أقسام : الأذن الخارجية ( الصيوان ومجرى السمع الظاهر ) الأذن الوسطى طبلة الأذن وعظيمات السمع ، قناة أستاكيوس وعظم الخشاء ) الأذن الداخلية : تتألف من التية العظمي LABYRINTH الذي يملؤه سائل يسمى اللمف المحيطي . ويتكون التيه العظمي من الأقنيه نصف الآليه والدهليز والقوقعة إن النسبة العظمى يتصل مع الدماغ بقناة تسمى القناة المائية القوقعية . ويتكون النسبة العظمى من الأقنية نصف الهلالية والدهليز والقوقعة إن النسبة العظمى يتصل مع الدماغ بقناة تسمى القناة المائية القوقعية يتصل بواسطتها اللمف المحيطي بالسائل الدماغي الشوكي
، لذلك فإن تركيب اللمف المحيطي يشبه تركيب السائل الدماغي الشوكي والسائل خارج الخلايا حيث أن الشاردة الرئيسية فيه هي شاردة الصوديوم . أما التية الغشائي فهو يقع ضمن التيه العظمي ويمتلأ التية الغشائي بالملف الداخلي ويشبه تركيبة السائل داخل الخلايا الشاردة الرئيسية هي البوتاسيوم . أما الأقنية نصف الهلالية الثلاث فهي تتوضع بشكل عامودي على بعضها تنتهي هذه الأقنية بخلايا حساسة للتوازن التي تتصل مع العصب الدهليزي الذي يسير مع العصب الحلزوني في مجرى السمع الباطن ليدخل الى الدماغ . أما وظيفة الدهليز فهي المساعدة على حفظ التوازن وذلك عن طريق منعكسات تجعل البدن في وضعية تحميه من السقوط فإذا مال الرأس إلى أحدى الجهتين أخذت الأطراف والجذع وضعية مناسبة بحيث لا يسقط الشخص وترد المعلومات عن وضعية أجزاء الجسم إلى الدماغ عن ثلاث طرائق هي العين والأذن والحس العميق . فالعين تعطي معلومات بالرؤية المباشرة والحس العميق يعطي بما يطرأ من شد على الأوتار والمفاصل والأذن وتعطي معلومات عن وضعية الرأس في الحركة والسكون . وننتقل إلى تعريف الدوار الذي هو شعور بدوران الجسم أو دوران الأشياء حول الجسم أي شعور خاطئ بوجود حركة لبنية بين الشخص والمحيط وهي نتيجة خلل في الجهاز الذي يحفظ توازن الجسم ، وهو يترافق مع شعور بالغثيان ، التعرق ، خفقان القلب . ومن أهم الأمراض المؤدية لحدوث الدوخة هو داء منيير وهو عبارة عن فرط توتر في الملف الباطن يشمل التيه الغشائي مما يؤدي التيه الغشائي مما يؤدي إلى تمدد وانتفاخ القوقعة وتبارزها داخل الدهليز . أما الأسباب الحقيقية لهذا المرض لاتزال مجهولة ولكن مع ذلك فقد تبين أن اضطراب استقلاب الماء وأحتباس شاردة الصوديوم في البدن والحساسية للهيستامين والتشنجات الوعائية والاضطرابات الهرمونية جميعها تؤدي لتغيير التركيب الكيميائي الحيوي للمف الباطن مؤدياً باللمف المحيطي لتحوله إلى داخل الملف الباطن مؤدياً لزيادة الضغط فيه مع تغير طبيعته الشاردية . وهو في الغالب مرض وحيد الجانب أي يصيب آذان واحدة عادة ولكن هناك حالات تصيب الأذنين معاً وهو يصيب الجنسين مع رضات قليل للذكور من أهم أعراضه . الدوار وهو العرض الرئيسي لهذا المرض ويأتي على شكل نوب من الدوار الشديد الفجائي حيث تستمر النوبة من دقائق الى ساعات . تترافق الغثيان والأقيـاء ثم تنتهي وهي تختلف من نوبه لأخرى حيث تأتي بمجموعات فيصاب المريض بعدة نوب متقاربة ثم تفارقه حتى يظن المريض نفسه بأنه شفى تماماً ثم تعاوده النوب مرة أخرى وقد تظهر الرأرأة ( حركة العين السريعة ) وتكون في الاتجاه المعاكس للأذن المصابة . نقص السمع : و هو من النوع الحسي العصبي و يكون تدريجيا ً و ذلك مع تكرار النوبة و يصبح دائما ً مع وجود لظاهرة الاستنفار و هو انزعاج المريض من الأصوات العالية. الطنين : و هو منخفض التواتر في البدء ثم يصبح مستمرا ً و قد يكون ظهور الطنين و نقص السمع تاليا ً لظهور الدوار بأشهر أو حتى سنوات . حس الثقل بالأذن : حيث يشعر المريض بحس ثقل و ضغط بالأذن المصابة . و يترافق مرض منير بأعراض إضافية و هي الغثيان و الاقياء و حتى الإسهال و أيضا ً الشحوب ، التعرق البارد و الصداع . حيث أن الشقيقة تترافق مع هذا المرض في 20% من الحالات و يشكو المريض من وجود مرض خطير بالدماغ و يعتمد التشخيص على الاستجواب الصحيح و القصة السريرية و ظهور الأعراض الأربعة مجتمعة أثناء النوبة . و تعتمد الفحوصات السمعية على تخطيط السمع حيث يظهر نقص سمع بإصابة الأذن الباطنة أم الأسباب الحقيقية لهذا المرض فمازالت مجهولة و لكن في بعض الحالات يمكن كشف عدة أسباب منها : – الحساسية لغذاء أو دواء معين – اضطرابات غددية كقصور الدرق – سوابق رض على الرأس – التهابات الأذن الوسطى المزمنة مع التهابات بالغشاء – اضطرابات مناعية أما التعريف فيكون المرض مشابها ً لالتهابات الدهليز و العصب الدهليزي ، الدوار الوضعي السليم ، داء التصلب اللايحي . أم العلاج فهو دوائي و جراحي العلاج الدوائي : يكون بطمأنة المريض و الشرح المعضل حول طبيعية المرض مع إعطائه المهدئات مع مضادات الدوار و الاقياء و كذلك إعطائه الموسعات الوعائية مع النصح للمريض بالابتعاد عن تناول الملح و ينصح بالابتعاد عن التدخين . العلاج الجراحي : يلجأ إليه في قليل من الحالات و في حال فشل العلاج الدوائي ز منها إزالة الضغط عن اللمف الباطن ، أو تمزيق الدهليز أو قطع العصب الدهليزي أو إعطاء بعض الأدوية المثبطة للدهليز مثل المبتاميسين . و لكن معظم العلاج الجراحي نتائجه غير مشجعه . و بالمحصلة فإن مرض منير مرض غير خبيث و لكنه مزعج جدا ً للمريض بحيث يقعده عن العمل و يؤدي به للخوف الشديد و لذلك على الطبيب أن يكون متفهما ً الحالة و يقوم بطمأنة المريض للوصول إلى أقصى درجات العلاج .
