من المعروف أن الاكتئاب يسبب آلاماً غير محددة، وكثيراً ما يذهب مرضى الاكتئاب إلى عيادات الأطباء العامين وأطباء الأسرة والمجتمع يشكون من آلام غير محددة وليس لها تفسير فسيولوجي.
في المؤتمر الثاني للطب النفسي العضوي الذي عقد هذا العام في شهر ابريل في مدينة سانتياجو عاصمة تشيلي، كانت هناك ندوة عن الألم والاكتئاب تحدث فيها نخبة من المختصين بهذا الموضوع المهم، للأطباء بوجه عام وللأطباء النفسيين على وجه الخصوص.
تحدث البروفسور دونكن ميتشل عن "الاكتئاب والألم" "Depression and Pain" وقال إن أكثر الأعراض المرضية التي يأتي بها مرضى الاكتئاب إلى العيادات العامة هي: آلام الجهاز الهضمي 85%، آلام الظهر 83%، الصداع 80%، آلام غير محددة في جميع أنحاء الجسم لا يستطيع المريض تحديد مكانها على وجه الخصوص، ضعف الطاقة وقلة الحيوية والنشاط، فقدان الرغبة الجنسية، تغيير في الشهية، اضطراب في النوم.
وقال إن مضادات الاكتئاب لها دور رئيس في علاج مثل هذه الآلام، ولكن ليست جميع مضادات الاكتئاب تنجح في علاج مثل هذه الآلام، وقال إن عقاقير مثل الإميتربتلين (Treptizol)، والفنلافاكسين (Venlafaxine) هي علاج جديد مضاد للاكتئاب من الجيل الثالث المطور، وكذلك دواء ليس من مضادات الاكتئاب ولكنه يخفف هذه الآلام مثل الترامادول (Tramadol) وقال إن هناك اختلافات في تحمل الألم بين الناس وتؤدي العوامل النفسية دوراً كبيراً في موضوع الاستجابة لمضادات الألم.
وتمت في الندوة مناقشة الألم العضوي والألم النفسي، ففي دراسة لمدة سبع سنوات عن أعراض الألم، تبين أن الآلام التي منشؤها عضوي تستجيب للعلاج وينتهي الألم بعد فترة زمنية قد تطول أو تقصر، ولكن الآلام التي منشؤها نفسي قد تستمر لأكثر من سبع سنوات من دون أن تستجيب بشكل جيد ويبقى أثرها موجوداً حتى مع استخدام الأدوية المضادة للألم. وتوصل الباحثون في هذا الموضوع إلى أن العلاقة بين الألم والاكتئاب مستمرة وذات علاقة وطيدة وهذا يشاهد في المرضى الذين يراجعون المستشفيات وكذلك عند عامة الناس.
هذا الأمر يفسر كثيراً مما نشاهده في مجتمعاتنا التي لا يحسن كثيرون التعبير عن مشاعرهم بصورة شفهية، وعندما يقع في مشكلة عاطفية أو عائلية تكون الشكوى من الآلم هي الحل حيث يكون هناك مسوغاً للتنقل بين عيادات الأطباء من عيادات القلب إلى الجهاز الهضمي إلى عيادات الأعصاب ولا تنفع الأدوية ومن ثم يبدأ مشوار المعالجين الشعبيين وهنا تبدأ دوامة لا تنتهي..!
بينما الأمر لا يتجاوز علاجات مضادة للاكتئاب قد تفيد بشكل كبير ولكن الثقافة والأعراف السائدة وقلة الوعي لها أثر كبير في خلق مثل هذه الدوامات من الدوران والضياع بين عيادات الأطباء وبيوت المعالجين الشعبيين وهذا أمر مؤسف حقاً..!!

سواء كانت هذه المشاكل تنحصر في الجهازز الهضمي فقط كما في بدايات الاصابة بالفيروس او في الحالات المتقدمة
والاخ الباحث كاتب الموضوع يعترف بأن هناك علاقة وطيدة بين هذان العرضان المرضيان .
ولكنه لم يخبرنا بنوعية تلك العلاقة وماهيتها .
