Pityriasis rosea is a rash of unknown causes that affects more women than men.
A skin disease characterized by the appearance of a crusty rash on the trunk and sometimes extending to the abdomen and extremities. It appears as pink spots at any age, but the highest percentage is between 15-35 years and it is one of the diseases whose source and causes are still unknown. Women are bigger and up to two versus one.
As for the rate of infection among children, it is very low, especially under the age of two years, and the rate of infection with the disease is more frequent in some seasons than others, especially in the early spring and autumn seasons, but with this it may appear in any season of the year.
Different reasons and opinions
There are conflicting opinions about the causes of roseola, as there is a theory that focuses on the fact that it is caused by a virus, but the nature of this virus is not yet clear. Attempts by many researchers to explore it have failed.
The justification for this is the inability to separate and analyze this virus, because its separation requires a certain technology that we did not recognize
There are other theories that focus on that the cause of this disease is one of the types of fungi that some researchers were able to discover in the crusts associated with the disease, but the question remains here, are these fungi the cause of the emergence of the disease or do they play a secondary role?
There is another theory that the cause of pityriasis rosea is streptococcus bacteria after researchers noticed that some cases occur after infection with impetigo. It may be a reason for the emergence of pityriasis rosea, while others see that the disease occurs as a kind of allergy to some types of drugs such as chloroquine and substances such as arsenic.
There is a belief that the disease may occur when wearing new clothes in a humid atmosphere, which leads to contamination with some insects that may help transmit the microbe that causes the disease.
Diagnosis
Although it is easy to identify the disease in the form of the rash and the areas of its distribution on the body, it is necessary to consult specialists in dermatology, since in some cases the shape and distribution of the rash is unconventional.
It should also be taken into account the work of tests for syphilis, as the rash in secondary syphilis is very similar to the rash found in pityriasis rosea. There are other skin diseases in which the rash resembles the rash found in pityriasis rosea.
And some medicines may cause a rash similar to that found in pityriasis rosea, and these medicines include arsenic, bismuth, metronidazole, and gold preparations.
There are cases that may last more than 8 to 12 weeks, which requires taking a sample from the site of the rash for the purpose of conducting a microscopic examination.
Symptoms and stages
Symptoms of the disease begin with the appearance of the mother infection called Herald patch, which is the first infection of the disease, and the place of infection is the trunk, thigh, arm and neck. Pinkish-red, surrounded by a distinct belt of small scales, and the center of the injury is yellowish-brown in color.
And the mother's injury precedes the appearance of the overall rash by a period ranging from (5 to 15 per day), but it may be much less than that, that is, up to a few hours only. We find that the global rash appears in batches between one and the other for up to three days, and it lasts up to ten days. Pityriasis rosea causes very annoying cases, including itching with high temperature sometimes, headache, lethargy, loss of appetite, intestinal malaise, and congestion in the throat with enlarged lymph nodes, and these General or limited to the cervical lymph nodes.
The infection sometimes reaches the scalp, hands, and legs, especially in children. It may occur in the oral cavity where the mucous membrane lining the mouth, and sometimes appears with bleeding spots under the skin and bubbles.
Disease complications
There are no serious complications of pityriasis rosea, but the severe itching that causes it most often leads to scratches in the skin, where it is susceptible to infection with a secondary bacterial infection, and the place of infection becomes dark in color that remains for several weeks and even months, but in some patients it leaves a trace For many years. The problem with this disease is that it is an infectious disease, but the possibility of infection requires prolonged contact, so it spreads in the family quickly and it is better to separate the infected from others for a certain time. The advice given to the afflicted is not to use a lot of loofah and soap, and to replace woolen clothes that cause itching with light cotton clothes.
Treatments
Since the cause of the disease is unknown, there is no specific treatment and the rash takes its time and goes away automatically, but if there is itching accompanying the rash, you can use palliative paints such as camphor oil and menthol. The most important thing is to take care of the skin by keeping it clean, and the other advice as long as the disease exists is to avoid bathing with hot water, rather warm water, and to avoid work that causes perspiration, sports and tight clothes made of synthetic fabrics because they increase itching, which in turn increases pimples, and wiping the skin after the bath with creams. Keep it greasy. But it is recommended to conduct a microscopic examination if the disease persists for more than two months, because it may be a sign of the presence of a difficult disease, and the patient should also avoid sunlight, because it irritates the skin and increases itching.

......النخالة الوردية.. طفــح جلدي أسبابـه مجهولـة يصيب النساء أكثر من الرجال.
مرض جلدي يتميز بظهور طفح قشري علي الجذع وأحياناً يمتد الى البطن والأطراف .يظهركبقع وردية في أية مرحلة عمرية ولكن النسبة الأعلى بين 15-35 سنة وهو من الامراض التي ما زالت مجهولة المصدر والاسباب، ويصيب مرض النخالة الوردية الرجال كما النساء، لكن نسبة الاصابة عند النساء اكبر وتصل الى اثنين مقابل واحد.
اما نسبة الاصابة لدى الاطفال فهي قليلة جدا خصوصا ما دون سن العامين، كما ان نسبة الاصابة بالمرض تكثر في بعض المواسم دون غيرها، خصوصا في اوائل فصلي الربيع والخريف ولكن مع هذا قد يظهر في اي فصل من فصول السنة.
أسباب وآراء مختلفة
تضاربت الاراء بشان اسباب مرض النخالة الوردية ،فهناك نظرية تركز على ان سببه هو الاصابة بفيروس، ولكن طبيعة هذا الفيروس لم تتضح الى الان. ولقد باءت بالفشل محاولات العديد من الباحثين لاستكشافه.
وتبريرذلك بعدم المقدرة على فصل هذا الفيروس وتحليله فان فصله يحتاج الى تكنولوجيا معينة لم نتعرف عليها
وثمة نظريات اخرى تركز على ان سبب هذا المرض هو احد انواع الفطريات التي تمكن بعض الباحثين من اكتشافها في القشور المصاحبة للمرض، ولكن يبقى السؤال المطروح هنا، هل هذه الفطريات هي سبب ظهور المرض او هي انها تلعب دورا ثانويا؟
وتوجد نظرية اخرى ترى ان سبب مرض النخالة الوردية هو البكتيريا العقدية بعد ما لاحظ باحثون ان بعض الحالات تحدث بعد الاصابة بمرض الحصف، وهذا المرض معد، وبالعادة يصيب الاطفال، وسببه البكيتريا العقدية او السبحية او كلاهما، بينما هناك من يرى ان وجود التهابات مزمنة باللوزتين قد يكون سببا لظهور مرض النخالة الوردية، في حين يرى اخرون ان المرض يحصل كنوع من الحساسية لبعض انواع الادوية مثل كلوركوين ومواد كالزرنيخ.
وهناك اعتقاد بان المرض قد يحدث عند ارتداء ملابس جديدة في جو رطب،مما يؤدي الى تلوثها ببعض الحشرات التي قد تساعد على نقل الميكروب المسبب للمرض.
التشخيص
مع أنه يسهل التعرف على المرض بشكل الطفح ومناطق توزيعه على الجسم إلا أنه يجب استشارة المتخصصين في الأمراض الجلدية و ذلك حيث أن بعض الحالات تكون فيها شكل الطفح وتوزيعه غير تقليدي .
كما يجب أن يوضع في الاعتبار عمل الفحوص الخاصة بالزهري وذلك حيث أن الطفح في الزهري الثانوي secondary syphilis يكون قريب الشبه جدا بالطفح الموجود في النخالة الوردية . وهناك أمراض جلدية أخري يشبه فيها الطفح الجلدي الطفح الموجود في النخالة الوردية .
وبعض الأدوية قد يسبب طفح جلدي مشابه للموجود في النخالة الوردية ومن هذه الأدوية الزرنيخ arsenicals والبزموت bismuth والمترونيدازول metronidazole ومستحضرات الذهب gold .
وهناك حالات قد تستمر أكثر من 8 إلى 12 أسبوعا ما يستدعى أخذ عينة من مكان الطفح بغرض عمل فحص مجهري .
أعراض ومراحل
تبدأ اعراض المرض بظهور الاصابة الام وتسمى Herald Patch وهي الاصابة الاولى للمرض ويكون مكان الاصابة هو الجذع والفخذ والذراع والرقبة، وتظهر الاصابة كبقعة بيضاوية الشكل يصل محيطها الى ما بين السنتمر الواحد والسنتمترين، ويلاحظ حجم الاصابة بصورة بطيئة كما يلاحظ ان اطراف منطقة الاصابة تكون ذات لون احمر وردي، محاطة بحزام مميز من القشور الصغيرة ومركز الاصابة مائل الى اللون البني المصفر.
والاصابة الام تسبق ظهور الطفح الجلدي الشامل بفترة تتراوح ما بين (ال5 الى 15 يوميا)، ولكن قد تكون اقل من ذلك بكثير اي بحدود عدة ساعات فقط. ونجد ان الطفح الجلدي الشامل يظهر على دفعات بين الواحدة والاخرى حتى ثلاثة ايام، وتستمر حتى عشرة ايام.ويسبب مرض النخالة الوردية حالات مزعجة جدا منها الحكة مع ارتفاع في درجة الحرارة احيانا وصداع وخمول وفقدان للشهية وتوعكات معوية واحتقان في الزور مع تضخم في الغدد اللمفاوية وهذه تكون عامة او محدودة بالغدد اللمفاوية العنقية.
وتصل الاصابة في بعض الاحيان الى فروة الرأس واليدين والرجلين، وذلك بصفة خاصة لدى الاطفال، وقد تحدث في تجويف الفم حيث الغشاء المخاطي المبطن للفم، وتظهر احيانا مع بقع نزيف تحت الجلد وفقاعات.
مضاعفات المرض
لا توجد اية مضاعفات خطيرة لمرض النخالة الوردية، لكن الحكاك الشديد الذي يسببه يؤدي في اكثر الاحيان الى خدوش في الجلد، حيث تكون عرضة للاصابة بالتهاب بكتيري ثانوي، كما يتلون مكان الاصابة بلون داكن يبقى عدة اسابيع وحتى اشهر، الا ان عند بعض المرضى يترك اثرا لسنوات طويلة.والمشكلة في هذا المرض انه من الامراض المعدية، ولكن امكانية العدوى تحتاج الى المخالطة الطويلة ، لذا فهي تنتشر في العائلة بسرعة ومن الافضل فصل المصاب عن الاخرين لوقت معين. والنصيحة التي تقدم للمصاب عدم استعمال الليفة والصابون بكثرة واستبدال الملابس الصوفية التي تسبب الحكة بملابس قطنية خفيفة.
العلاجات
وبما أن السبب غير معروف للمرض فلا يوجد علاج معين والطفح يأخذ وقته ويزول تلقائياً ولكن إذا كانت هناك حكة مصاحبة للطفح فيمكن استعمال دهانات ملطفة كزيت الكافور والمنتول ، أما اذا كانت الحكة شديدة فيمكن استعمال كريمات الكورتيزون وللتهدئة ليلاً تستعمل أقراص مضادة للهيستامين قبل النوم الا انه يعتبر ان الاهم هو العناية بالجلد عبر الحفاظ على نظافته، والنصيحة الاخرى طالما ان المرض موجود يجب تفادي الاستحمام بالماء الساخن بل الدافئ وتفادي العمل الذي يسبب عرقا والرياضة والملابس الضيقة المصنوعة من الانسجة الاصطناعية لانها تزيد من الحكاك الذي يزيد بدوره من البثور، ومسح الجلد بعد الحمام بكريمات تبقيه دهنيا. لكنه ينصح باجراء فحص مجهري اذا ما بقي المرض اكثر من شهرين لانه قد يكون علامة لوجود مرض صعب، وعلى المريض ايضا تفادي اشعة الشمس لانها تهيج الجلد وتزيد من الحكاك.
